Mario's wheelchair squeaked against the concrete as he positioned himself in front of me. Six feet away, Sirena stood filming on her phone, though her son hadn't taken his first step yet. He hadn't even tried. I knelt on the floor of their Port of Spain home, the Jaipur Foot prosthetic laid out in pieces beside me: lightweight rubber, aluminum connectors, foam padding. The chamber microphones voiced what had started in a concrete room. For a moment, advocacy felt like progress: words building the structures that tools could not. But when the applause faded, a delegate stopped me in the hallway and asked quietly, "How will you scale this?" I answered with more confidence than I felt. He nodded, but the question followed me home.

Each experiment demanded steadier hands and smaller margins, the tremor of a pipette tip threatening to undo weeks of work. The day it hit that mark, the room celebrated, then went still. Someone asked, "Can clinics there even run this?" I heard echoes of every question before it.
